Football Registration Fees** All Fees are due at time of Signup:
$130
Registration (2nd child $110 - 3rd child $90)
$50/ Child Fundraisers (all fundraisers will
be optional after this payment)
$20/ Child Chains & Concessions; refundable if coaching, concession
or chains worked
**Additional fees will include: Football Jerseys (payment due at time of order); other necessary
gear Cheerleading

*2011-2012 Pop Warner Physical Form (a physical clinic will be offered this year cost is $25/physical)
*Signed Code of Conduct Agreement
*Copy of 2010-2011 Report Card (4 full semesters) COACHES ARE NEEDED
